ENDOMETRİOMA NEDENİYLE LAPAROSKOPİK CERRAHİ GEÇİREN HASTA GRUBUNDA AGONİST VE ANTAGONİST TEDAVİ PROTOLLERİNİN IVF PROGNOZUNA ETKİSİ*

Abstract

Endometriosiz kronik, yaygın, östrojen bağımlı inflamatuvar bir hastalıktır. Bu hastalığın kesin prevalansı bilinmemekle birlikte üreme dönemindeki kadınları %2-10 oranında etkilediği tahmin edilmektedir. Bu çalışmada endometrioma nedeniyle laparoskobik cerrahi geçiren hastalarda uygulanan agonist ve antagonist protokollerin İVF prognozuna etkisinin belirlenmesi amaçlanmıştır. Buna göre Ankara Etlik Zübeyde Hanım kadın hastalıkları ve eğitim araştırma hastanesinde 2007-2015 yılları arasında endometrioma nedeniyle IVF kliniğinde tedavi programına alınan hastaların dosyaları taranmıştır. Endometrioma tanısı alan ve bu sebeple opere olmuş 64 siklus çalışmaya dahil edilmiştir.
Çalışmada agonist ve antagonist tedavi grubu olarak ikiye ayrılan toplam 64 hastanın bazal 3. gün FSH düzeyi antagonist grupta agonist tedavi grubuna göre çok önemli derecede anlamlı bulunmuştur. Stimülasyon parametrelerinden GnRH kullanım süresi agonist tedavi grubunda antagonist tedavi grubuna kıyasla istatistiksel olarak çok önemli seviyede uzun bulunmuştur. Diğer taraftan, fertilizasyon oranı agonist grupta antagoniste göre istatistiksel olarak yüksek bulunmuştur.
